About the Center

Crescent Ridge opened its doors in 1991.  Since that time, we have served hundreds of seniors in need of day-time care, socialization and medical supervision.  During the past several years, we have added services for younger people with health care needs as well as for our veterans.

The Center serves up to 40 participants a day. Hours of operation are Monday through Friday, from 7:30 AM to 6:00 PM. Time of arrival depends on transportation schedules arranged by the Center, or drop off times arranged by the family. Attendance may range from 2 to 5 days a week.

Transportation is available in our comfortable, wheelchair accessible vehicles. Our safety certified and caring drivers will assist each participant to and from their front door. Transportation is available within a fifteen mile radius of the Center.

Services include: nursing/medical care, health education, medication management, rehabilitative care, recreational therapy, family education and support services, information referral, community integration, nutrition services and transportation.

Staffing consists of trained health care professionals, including a full time registered nurse, certified nursing assistants, recreational therapists, a social worker, program assistants, drivers, a consulting medical director, and a dietitian.
